Department of Transportation (DOT/CDL – Truck Driving) Physicals & Testing
Here at Dr. Jacques Charles Aesthetics Health & Wellness Institute we perform Department of Transportation (DOT/CDL) physicals which are required by your state in order to be allowed to drive certain motor vehicles.
When arriving at our office we ask that you provide any exempt letters/notifications from your primary doctor and bring any medications you are currently taking. We also need yout to bring any hearing aids, glasses, contact lenses, along with your ID and Driver’s License.
Be advised that a urine sample will also be required to complete the physical. The sample is not for drug testing purposes. All candidates are asked to arrive well hydrated so that when giving a urine sample it does not take a lot of your time.
All DOT physicals will have a fee. If drug testing is required, we can offer this extra DOT testing for an additional fee when performing it with the DOT physical.
